5. Hawaii Keiki Digital Consent (For your child to receive free Hawaii Keiki Nursing Care at school). 

For Returning Students to Kalani High School

* For SPAV, by checking NO, student's name and picture WILL NOT be in the yearbook, sports rosters, or graduation program.
